Provider Identification Badges

All eligible IHSS Providers can request and receive an IHSS Provider identification (ID) badge. To obtain an IHSS Provider ID badge, follow the steps below.

  1. Visit one of the following locations:
    Department of General Services – Security Office
    5560 Overland Ave., Ste. 210
    San Diego, CA 92123

    Same-day Service

    Public Authority

    401 Mile of Cars Way, Ste. 200
    National City, CA 91950
    (866) 351 – 7722
    Please note: The Public Authority location cannot print ID badges; your ID badge will be sent by mail at a later date.
  2. Fill out the Provider ID Badge Registration Form at one of the locations
  3. Provide an official unexpired US Federal or State valid photo ID
  4. Bring your most recent IHSS Provider paystub
  5. Have your picture taken at one of the locations
  6. You will receive your ID badge based on the following:
    – If you request your badge at the Security Office location, you will receive your ID badge same day
    – If you request your badge at the Public Authority office, staff will submit the form on your behalf, and you will receive your ID badge in the mail
